051101 - Completed initial preparation and got a temporary crown.  The next
051102 - appointment is in two weeks, to test the fit of the permanent crown.
051103 - Then the next day a final installation will be made.

051105 - Steve said his investigation during preparation of the tooth today,
051106 - does not indicate the crack observable above the gum line, extends
051107 - into the nerves.  He feels there is a good chance that root canal will
