- Walked across the hall to Cath Lab.
- ..
- Went in and talked to staff.
- ..
- A nurse checked the computer for scheduling ablation procedure today.
- She said nothing is scheduled.
- ..
- On further discussion, she found the procedure scheduled for today.
- ..
- Followed the nurse to the elevator; went up to 3rd floor, and into
- ASU.
- ..
- Nurse Renea took vitals. Blood pressure seems low (like 110 60) and
- HR high (like 70). Dressed in hospital gown for the procedure.
- ..
- Clothing, computer, keyboard, cellphone were inventoried and placed
- in large plastic bag, which was sealed with a strap.
- ..
- Cath Lab nurse walked me to another department to get weight.

- While we were talking Adam arrived. He assists Doctor Pellegrini with
- ablation procedures in the Cath Lab. Adam asked about patient history
- of hiiking and onset of paroxysmal atrial fibrillation that happened
- on 191210, reported in case study on 190101 0730. ref SDS 88 YTWS
- ..
- Adam indicated there is a better chance of success with ablation in
- this case, because it is being performed within months after PAF
- symptoms first occurred, rather than years after onset of disease.
- ..
- While discussing the ablation procedure and patient history, Doctor
- Pellegrini arrived.
- ..

- Pulmonary vein isolation
-
- https://www.mayoclinic.org/tests-procedures/pulmonary-vein-isolation/about/pac-20384996
-
- Pulmonary vein isolation is a procedure used to stop abnormal
- electrical signals in your heart that cause heart rhythm
- problems.
- ..
- Pulmonary vein isolation is a type of cardiac ablation.
- Cardiac ablation works by scarring or destroying tissue in
- your heart that triggers an abnormal heart rhythm. In some
- cases, cardiac ablation prevents abnormal electrical signals
- from traveling through your heart and, thus, stops the heart
- rhythm problem.
- ..
- In pulmonary vein isolation, the procedure creates scar tissue
- in the part of the left upper chamber of your heart where each
- of your four pulmonary veins connects. Your pulmonary veins
- bring oxygen-rich blood from your lungs to your heart.
- Pulmonary vein isolation can reduce the signs and symptoms of
- atrial fibrillation, which affects the upper chambers of the
- heart.

- ..
- Doctor Pellegrini said the CT test yesterday on 200429 0700,
- ref SDS G5 Y64H, shows a possible blood clot in the heart. Ablation
- cannot be performed with this condition. The doctor wants to do the
- TEE procedure, which was cancelled yesterday. It will confirm if
- there is a blood clot. If TEE does not show a blood clot, then the
- doctor will do ablation today.
- ..
- Adam advised TEE is similar to EGD procedure using a gastroscope to
- examine and treat the esophagus and stomach. Lidocane will be given
- to numb the esophagus. Findings from TEE may be sufficient for Doctor
- Lee to cancel EGD scheduled next week on 200505.

- A transesophageal echo (TEE) test... uses a long, thin, tube
- (endoscope) to guide the ultrasound transducer down the
- esophagus
-
- ...the doctor see... the heart without the ribs or lungs
- getting in the way. A TEE is done when your doctor needs a
- closer look at your heart
- ..
- Seemed like 5 or 6 people in the room. There was discussion about
- doing TEE test there in ASU or downstairs in the Cath Lab. Doctor
- Pellegrini decided to do the TEE in the Cath Lab. If the test is
- negative, she can immediately do the ablation procedure, without
- further moving the patient. If there is a blood clot, the patient can
- be moved to Med-Surg for recovery and discharge. In that case, will
- schedule ablation for weeks or months later, after treatment to
- resolve blood clot in heart. This would be a major disappointment.
- ..
- Still on the gurney was pushed out of ASU and into an elevator. The
- entire team walked back to 2nd floor and into Cath Lab.
- ..
- There was discussion whether to move patient from gurney onto Cath Lab
- table to perform TEE, or do TEE with patient still on the gurney? It
- was very crowded in the room at this time. Seemed like 8 or so
- people, plus the procedure table, and the gurney, which came from ASU.
- ..
- Was decided to do TEE on the gurney. If results are negative, patient
- will be moved from the gurney onto the Cath Lab table for performing
- ablation.
- ..
- Doctor Michela used a probe to squirt lidocane into the mouth. Had
- initial difficulty gargling. After several tries, where the doctor
- squirted additional lidocane, she was satisfied gargling was
- sufficient. Seemed like throat felt more numb (thick) than usually
- occurs with EGD procedures.
- ..
- The doctor applied the anesthesia drug.
- ..
- Was asleep for TEE and then ablation procedure.

- caused severe, extreme and continuous coughing and hacking, and
- delirious shouting out profanities, as if dreaming. This occurred
- througout the day and night, with brief interludes, when the patient
- slept.

- Machele was the nurse. Did a great job, until shift change, when
- Nadine took over. Nadine was terrific as well. Both very helpful.
- ..
- Adam advised TEE was negative for blood clot. Doctor Pellegrini then
- performed ablation, and has indicated this was very successful
- procedure.
- ..
- Think Doctor Pellegrini came into the room briefly. Confirmed Adam's
- understanding that the procedure met all criteria and without
- complications. Good likelihood AF will not recur, for reasons Adam
- presented earlier in ASU, per above. ref SDS 0 F861
- ..
- Stayed in bed rest of the day feeling very tired and weak. Did not

- Filed Under: AF ablation, Atrial fibrillation
- ..
- AF ablation is a big deal:
- ..
- The first thing to say about AF ablation is that it is a big
- procedure. I tell patients to expect AF ablation to be hard on
- them. How could it be easy to undergo hours of general
- anesthesia, insertion of big tubes in the leg veins, 50-75
- burns in the atrium and hours of bed rest?
- ..
- Resting heart rate changes:
- ..
- The resting heart rate can increase in the weeks or months
- after ablation. The increase is usually 10-20 beats per minute
- more than pre-procedure levels. This phenomenon usually
- resolves.
- ..
- Exercise Tolerance:
- ..
- The ability to sustain exercise can be decreased for weeks to
- months after the procedure. It will likely come back. The
- reasons AF ablation transiently decreases exercise tolerance
- are numerous: it is a big procedure, the burns can cause
- stunning of the atria, there is deconditioning that occurs
- before and after the procedure, and many patients remain on AF
- drugs for a month or two after the procedure.
- ..
- Digestive problems:
- ..
- I have seen, in a handful of cases, patients have trouble
- moving food through their GI tract after ablation. Symptoms
- include reflux, feeling bloated, and intolerance of big meals,
- while signs may include distention of the abdomen.
- ..
- Ablation in the left atrium may transiently damage nerves that
- control motility of the GI tract. The good news is that these
- effects resolve with time. I usually treat this problem by
- recommending small meals, and an acid blocker.
-
- ..
- We arrived in the room after lunch was served, but an attendant
- brought a lunch tray. Did not feel like eating anything.
- ..
- Similarly, dinner tray was delivered OA 1700. Could not eat
- anything.
- ..
- In late PM, nurse removed dressing from puncture wounds where ablation
- access occurred on both upper legs. The wound on right leg bled
- continuously. Several nurses tried for about 15 minutes each stopping
- the bleeding holding gause on the wound.
- ..
- Doctor David Aderson was summoned from home to stop the bleeding. He
- was successful using a combination of lidocane and another agent.
- This took half an hour with the doctor holding pressure on the
- bleeding wound. The doctor remembered this case from doing treadmill
- stress test on 200103 0700. ref SDS C9 2J4H
- ..
- Was feeling very ill prior and during this procedure. Thought it
- might be due to not having eaten for 30 hours or so.
- ..
- Doctor Anderson suggested a nurse place a cold compress on my
- forehead. After this was done, felt better. Was able to sleep for an
- hour or so.
- ..
- Talked several times with Kathy.
- ..
- Called and left message for Sally.
- ..
- Got a call from Rob. Talked for about an hour late at night. He
- related again his friend had success after ablation getting back on
- his bike to do long-mileage cycling.
