- and immunity, reported in connection with guidelines recently released
- by the American Heart Association AHA. ref SDS 17 NF9H
- ..
- On 131125 0005 research indicates Atorvastatin 10 Ezetimibe 10
- Increase HDL 5% TG 30% Lower LDL-C 50%. ref SDS 18 PU6I
- ..
- On 131125 0005 research in Drugs.Com indicates Ezetimibe less common
- side effects include Dizziness, Dryness or soreness of throat,
- Hoarseness, which have all occurred and may compound side effects of
- Atorvastatin. ref SDS 18 JO6K
- ..
- On 131125 0005 research indicates Tim Russert, popular television news
- commentator died from heart attack despite achieving reduction of
- cholesterol LDL 68, below the "gold standard" for coronary health;
- yet, his HDL 37 remained very low, and he continued overweight
- (metabolic syndrome). ref SDS 18 RK6O
- ..
- On 131125 0005 research indicates coronary CT angiography test
- measures calcium present in the arteries. Tim Russert had the scan
- done in 1998 with a score of 210. It should be 0. ref SDS 18 N73K
- Coronary computed tomography angiography (CTA) allows coronary artery
- visualization and the detection of coronary stenoses. In addition; it
- has been suggested as a novel, noninvasive modality for coronary
- atherosclerotic plaque detection, characterization, and
- quantification. ref SDS 18 VW3W
- ..
- On 131125 0005 research indicates HDL > 60 are associated with a
- reduced risk of coronary artery disease (CAD). HDL "scour" the walls
- of blood vessels cleaning out excess cholesterol that otherwise might
- have been used to make the plaques that cause CAD. ref SDS 18 4E6H
- ..
- On 131125 0005 research indicates clinical trials found increasing HDL
- with drugs does not yield benefits of regressing atherosclerotic
- plaques. ref SDS 18 KG9F
- ..
- On 131125 0005 research indicates aerobic exercise (walking, jogging,
- biking, that raises heart rate for 20 to 30 minutes at a time) may be
- the most effective way to increase HDL levels in a way that appears to
- lower risk of heart disease by regressing atherosclerotic plaques in
- blood vessels. The duration of exercise, rather than intensity (i.e.,
- speed) is the more important factor in raising HDL choleserol.
- ref SDS 18 SH4J
- ..
- On 131125 0005 research indicates overweight patients can increase HDL
- and lower LDL-C by losing weight. ref SDS 18 SH63
- ..
- On 131125 0005 research indicates LDL cholesterol occurs with two (2)
- protein cores, apo B or apo A1 (see diagrams). Apo B molecules are
- very small and highly associated with causing vascular disease. Apo
- A1 molecules are large and fluffy and much less likely to cause
- problems. ref SDS 18 1V4J
- ..
- On 131125 0005 research indicates small, dense LDL may increase risk
- for any given level of LDL. Studies suggest having small, dense
- particles may have up to 300 percent greater risk than having large
- LDL particles. ref SDS 18 PC5N
- ..
- On 131125 0005 research indicates Small, dense LDL correlates with
- high triglycerides (TG) > 140 and low HDL < 40. ref SDS 18 PD5I
- Triglycerides are lowered by exercise and weight loss, which also
- converts small, dense LDL particles into large, fluffy particles.
- ref SDS 18 PD69 Low TG < 70 indicates large, fluffy LDL particle size
- that cannot penetrate endothelium lining of blood vessels to cause
- atherosclerotic plaques. ref SDS 18 PD79
- ..
- On 131125 0005 research indicates statin drugs do not change LDL
- particle size appreciably. TG between 70 and 140 for patients with
- CAD consideration may be given to measuring LDL particle size.
- ref SDS 18 PD85
- ..
- On 131125 0005 research indicates consistent exercise lowers
- triglycerides, which are simply "fats" that circulate in the blood
- bound to particles called lipoproteins that also carry cholesterol.
- As they circulate, tissues pick up triglycerides to use for energy or
- store them to use as fuel later. Aerobic exercise for at least twenty
- minutes per session, preferably thirty or more minutes. For the first
- minutes of exercise, your body burns mostly glycogen, but if you
- exercise longer, your body switches to burning mostly triglycerides as
- fuel - if you keep the pace moderate. ref SDS 18 T68I

- ..
- Previous meeting with Doctor Wisneski on 151005, further disucssed
- purpose of CCTA. ref SDS 44 HE5Y
- ..
- Doctor Wisneski offered helpful suggestions on controlling statin side
- effects by dose reducing. ref SDS 44 GL7G
- ..
- Initially, Doctor Simpson asked about Hoka shoes, worn this morning
- for hiking at Lafayette Reservoir on the way home?
-
- [On 160504 1000 Doctor Simpson asked about buying new Hoka
- shoes to replace the Hoka shoes he has that are wearing
- out. ref SDS 57 MO91
- ..
- Explained background reported on 140708, buying over 20 pair of shoes
- the past 4 years or so, attempting to find shoes that protect against
- feet injuries, primarily metatarsalgia, beginning on 140627 1223.
- ref SDS 22 0001 In 2013 until 140708, this included resoling existing
- shoes that previously protected against feet injuries. ref SDS 23 PPUV
- On 140708, received letter from the owner of Resole America,
- suggesting better solution would be to buy Hoka shoes. ref SDS 23 XN5F
- ..
- On 140713 1640 bought the most expensive Hoka Conquest shoes from
- Fleet Feet shoe store in Pleasant Hill. ref SDS 24 JY4O These shoes
- caused severe blisters, and so were replaced by Fleet Feet at no
- charge with Hoka Simpson Trail shoes on 140726 1030. ref SDS 25 LE7M
- However, the heels failed on these shoes, so Hoka replaced them again
- at no charge with Hoka Stinson Lite shoes on 141029 1627. ref SDS 30
- XN5F These shoes turned out to solve blister problems, and more
- importantly the high level of cushioning resolved in combination with
- Prolab orthotic inserts the metatarsalgia problems, reported on 141026
- 1617. ref SDS 29 GU5N
- ..
- Doctor Simpson advised that he also runs and hikes distances. He has
- had success resolving blisters using...
-
- Burt's Bees foot cream
-
- [On 151113 1130 meeting Doctor Tang in Podiatry Department;
- examination shows Geri-Hydrolac 12 (Ammonium Lactate 12%),
- and Burt's Bees foot lotion recommended in Cardiology
- Department not completely successful preventing blisters on
- big toes both feet caused by dehydration due to frequent
- urination caused by Omeprazole (PPI) 20 mg and statin
- treatments with Avorstatin 10 mg and Ezetimibe 10 mg taken
- daily. ref SDS 52 AW6H
- ..
- [On 160504 1000 Doctor Simpson asked about buying new Hoka
- shoes to replace the Hoka shoes he has that are wearing
- out. ref SDS 57 MO91
- ..
- Agenda for the meeting this morning is shown in the letter to Doctor
- Rao, with copy to Doctor Simpson on 150730 0952. ref SDS 37 UR51
- ..
- Doctor Simpson advised that he does not recall receiving this letter.
- He asked why he was sent a copy of the letter on 150730?
- ..
- Reviewed background getting recommendation that Doctor Simpson might
- take an interest in research/study to manage CVD risk through
- treatment with extended endurance exercise, measured with labs and
- CCTA that assess response to treatment, based on the spreadsheet
- submitted in a letter on 150728, also cited in the letter with copy to
- Doctor Simpson on 150730 0952, ref SDS 37 UR51
- ..
- Doctor Simpson was aware of patient history having CABG x4 heart
- surgery on 091022 0700. ref SDS 4 PQWU At that time the medical team
- reported favorable conditions for successful bypass surgery in that
- bypass grafts were of "good quality," shown in the record on 100928
- 0706, ref SDS 12 YI7K
- ..
- Today, the doctor requested background on symptons that required
- surgery on 091022?
- ..
- Explained meeting Doctor Lee to continue treating Achalasia in the GI
- Department at the VA Clinic in Martinez, reported on 090908 1130.
- ref SDS 1 MY4N Treatment to resolve achalasia was delayed 3 - 4
- years, which caused esophagus to expand, possibly pressing against
- lungs. After 6 years hiking 3-mile trail at Lafayette Reservoir,
- sometimes 2, 3, 4 even 5 laps, i.e., 14 miles, in September 2009,
- patient had chest pains hiking up the first hill at Lafayette
- Reservoir - see background on Achalasia also listed in the same record
- on 090908. ref SDS 1 FT6M) Doctor Lee ordered treatment for
- Achalasia, and ordered a treadmill stress test to evaluate chest
- pains, since no prior incidents had been recognized the prior 6 years.
- On 091002, Doctor Eagen in Cardiology diagnosed results from treadmill
- test indicated likely triple vessel disease. He made referral for
- Cardiac Catheritization (angiogram test) here at the VA Medical Center
- in San Francisco. ref SDS 2 V86G
- ..
- Doctor Simpson asked the purpose of meeting today?
- ..
- Purpose of meeting today is to order standard of care testing for
- response to treatment of atherosclerosis that supports reducing
- treatment in order to reduce debilitating side effects. Current
- strategy is to test with coronary CT angiography (CCTA) and include
- calcium score, discussed with Doctor Stewart in connection with
- reducing side effects of PPI (Omeprazole) prescribed to treat
- achalasia symptoms of ulceration on LESV discovered in EGD dilation
- procedures, reported during a meeting on 150623 1300. ref SDS 34 1Y5L
- At that time, on 150623, Doctor Stewart supported ordering CCTA and
- deferred to Primary Care, shown in Progress Notes. ref SDS 34 UM4O
- ..
- On 140213 0830 meeting at VA considered performing coronary CT
- angiography (CCTA) test with calcium score to evaluate regression of
- atherosclerosis, ref SDS 20 JZ4O, based on labs showing HDL 30 when
- CABG +4 was performed in 2009, to HDL 62, as well triglycerides (TG)
- lowered significantly, indicating favorable conditions for regression
- of atherosclerotic plaques, and as a result of hiking 11 miles per day
- sometimes 300+ miles per month the past few years, citing research on
- 131125. ref SDS 18 6O9M
-
- ..
- Subsequently on 150209, conditions for regression of atherosclerotic
- plaques improved with HDL 70. ref SDS 31 5C7M During patient review
- meeting on 150213, with Doctor Rao, following patient cardiology care
- with Atorvastatin 10 mg and Ezetimibe 10 mg, the doctor noted that
- raising HDL 130% from 30 on 091022, when patient suffered severe
- atherosclerosis with CVD that required CABG x4, to reach HDL 70,
- reflects hiking 10,000 miles the past 4 years. ref SDS 32 ZM3I
- Despite raising HDL from 30 to 70, Doctor Rao objected to ordering
- CCTA test to evaluate favorable effects of this change. ref SDS 32
- L85J
- ..
- Doctor Simpson commented that years ago drugs were developed that
- reached HDL 90; however, this did not achieve regression of
- atherosclerotic plaques. This aligns with research finding that
- cardiac risk increased using drugs to increase HDL, reported on 131125
- 0005. ref SDS 18 KG9F
-
- [On 151204 1400 telecon with Doctor Vernaccia noted success
- raising HDL with drugs, but that this form of HDL failed to
- regress atherosclerotic plaques and failed to extend
- longevity. ref SDS 54 MQ7I
- ..
- [On 160405 1305 large 12000 patient study found the drug
- evacetrapib lowered LDL 37 percent and increased HDL by 130
- percent; however, study was abandoned, because findings
- showed risk for adverse cardiovascular health events was
- not affected. ref SDS 56 J47N
- ..
- The same article "Raising Your HDL Levels," maintains that increasing
- HDL with aerobic exercise for 20 - 30 minutes has significant health
- benefits, also shown on 131125 0005. ref SDS 18 SH4J Another article
- reported similar findings that increasing HDL with drugs did not
- improve outcomes, while increasing HDL with exercise does improve
- health outcomes. ref SDS 18 ZT58 Pubmed published another article
- correlating high HDL through exercise with lowering CVD risk.
- ref SDS 18 U98N
- ..
- Better outcomes from increasing HDL with exercise may be explained
- from research showing this causes "rapid regression" of
- atherosclerotic plaques. 131125 0005, ref SDS 18 KU6J

- He is doing extremely well w/o any CV sxs, esp no angina.
- Lipid panel is very good, and HDL is high, which he attributes
- to his vigorous exercise regimine. However, he feels that
- statin rx is causing xs urination and dry/cracked feet, and he
- would like to continue not taking it (he is not taking a statin
- now). He feels that a good CTA will support that, whereas a
- bad CTA would require further work.
- ..
- 9. PLAN:
-
- We had a very long discussion of the pros and cons. In my
- opinion, a "good" CTA would not rule out unstable and/or
- non-calcified plaque that might be prone to rupture and MI, and
- might be stabilized by statin rx. On the other hand, a "bad"
- CTA might present a quandry of what to do in a pt who is doing
- so well clinically. I also said that I don't think CTA is
- sufficiently sensitive to follow CAD, esp over short times, not
- to mention the xs radiation exposure would make this not
- indicated. Guidelines do not recommend this application.
- ..
- Nevertheless, he is very intent on getting this test, and I
- have ordered it. We will reassess after. Obviously we can
- adjust statin rx, although I doubt that statin is responsible
- for urinary frequency and/or dry feet.

- I agree with above. CTA is not a sensitive test to follow
- coronary lesions. Patient is doing very well but with his
- prior hx--indication for statin therapy is strong--- need to
- find a statin and dose of statin that he can tolerate. Patient
- has stopped me in hallway and I had similar discussion with him
- that CTA is not recommended for this purpose.
