-
- This morning, submitted letter to Doctor Rao wiith agenda for meeting
- today...
-
- 1. Agenda Meeting Today Resolve Injuries Fall Half Dome Fall
- Date: 2015-09-23 0922
- ..
- 2. Your letter on 150921 1217, set work plan for a member of the
- MP PACT team to call. Since no one called, later that day on
- 150921, drove to VAMCSF and met with Lisa in your office OA
- 1530. I had hoped to be admitted for exigent care of rising
- symptoms, principally pain, as explained in my letter the next
- day on 150922 0502. ref SDS 10 XQ71
- ..
- 3. Since Monday, pain has increased significantly.
- ..
- 4. From laying down or sitting for more than 10 minutes, then
- standing up (where the leg goes vertical) causes pain to rise
- to unbearable level at the point of the wound even without load
- bearing weight on the floor. Touching the floor with the left
- foot, causes pain to rise instantly. Using crutches is very
- difficult, but essential to maneuver. This effort to stand up
- the past few days has caused several falls, which increases
- pain from the fall and then getting up.
- ..
- 5. Once fully vertical, crutches enable walking with just one
- foot. After a few minutes, the body adjusts and pain subsides
- allowing some weight to be transferred to the left foot onto
- the floor, and then in another 60 seconds or so, can walk
- normally with pain level 2 - 4.
- ..
- 6. Walking is well tolerated at this pain level. However, if I
- stop and stand still for more than 10 seconds or so, pain level
- quickly rises to "12+". Thus, I have to keep walking, which
- may be a good exercise for recovery, as related in the meeting
- with Doctor Turner in the ER on 150917. ref SDS 7 4N6Q
- ..
- 7. These are significant changes. Previously did not need
- crutches; got up and walked without pain, and could stand
- without pain for first 4-5 days after the initial fall on
- 150918. As well, had not fallen down while trying to stand up
- until Monday.
- ..
- 8. This record seems to suggest there is increased blood flow to
- the wound when changing to a vertical position (i.e.,
- standing). Further there is increase accumulation of blood at
- the wound when standing. Guessing increased blood flow to the
- wound causes increased pressure on damaged nerves that
- registers as pain. Walking enables blood flow to resume,
- relieving pain.
- ..
- 9. Yesterday, walked 130 laps between dining and living room,
- about 2 miles. In a few minutes will try regular hike outside
- for 3 - 6 miles, then drive to the Medical Center for the
- meeting.
- ..
- 10. Issues to consider...
- ..
- 11. Admit patient to prevent further falls, perform medical review,
- consultations, procedures, and assessment of initial recovery.
- ..
- 12. Radiology studies determine scope of injuries.
- ..
- 13. Drain wound - needle aspiration.
- ..
- 14. Compression dressing on wound might prevent pain from
- escalating to level 10 on initial standing up, and standing
- still after walking.
- ..
- 15. Look forward to seeing this afternoon.

-
- Plenty of time before meeting at 1530, so hiked to the beach on
- Highway 1, about 1.5 miles from the VA on Highway 1, and South of the
- Cliff House. Hiked a short distance from the road toward the water
- (Pacific Ocean). Decided not to hike out to the water, because it was
- quite a distance in soft sand, so going out and coming back would be
- very slow.
- ..
- Hiked back to the VA and arrived in time for meeting with Doctor Rao

- Doctor Rao had not had time to review the letter submitted earlier
- today, presenting rising symptoms, per above, ref SDS 0 JN3J, and with
- the agenda for the meeting today, per above. ref SDS 0 XR82
- ..
- Handed the doctor a printed copy of the letter to read in order to
- prepare for a productive meeting.
- ..
- Doctor Rao read the letter.
- ..
- She examined the wound on the lower left leg. The doctor checked
- blood vessels on inside of both ankles. She expects severe pain in
- the left leg hematoma is not due to blockage in blood flow, based on
- feeling the rate of flow seems the same in both legs, and is normal.
- ..
- Doctor Rao proposed calling General Surgery Department to request
- immediate examination this afternoon to evaluate need for surgical
- intervention treating rising symptoms, shown in the letter today, per

- talked to Isagani.
- ..
- Doctor Rao said she has to leave early today. She explained Medical
- Practice Department closes at 1700, but the patient waiting area
- remains open for another hour or so while nursing and other staff
- complete daily administration.
- ..
- The doctor suggested waiting for the representative of General Surgery
- to arrive in the Patient Waiting area to make an initial examination
- of the wound, and decide on line of care, if any, for the Surgery
- Department. She will notify the Medical Practice staff that the
- patient is authorized to wait in the Waiting area after hours until
- the examination by Surgery.

- pt fell while hiking Yosemite 1 week ago and hit left leg,
- hands (no loc, no trauma to head). He continued to hike
- subsequently, but then stopped when pain worsened. Seen in ER
- 2 days later-- vasculature intact, recommended non-wt bearing,
- activity OK. Pt continued to walk (and in fact, has been
- walking for miles daily...up to 10 miles on 1 day). On monday,

- order to prevent and disolve blood clots in lieu of draining the

- days. When hiking there is no pain, but when getting up from a
- vertical position on bed or sitting with leg elevated, e.g., working
- in a chair with leg rest, there is very severe pain. At first this
- lasted only a few seconds. But a day later it took several minutes,
- and now it takes up to 5 minutes for pain to subside enough to put
- weight on the leg, and then to walk, as set out in the letter
- submitted to Doctor Rao in VA Medical Practice, earlier today, per
- above. ref SDS 0 XR60
- ..
- Debra the nurse set an IV in the right arm.
- ..
- The doctor said the patient will receive Dilaudid (hydromorphone)
- through the IV to ease pain, but this will not induce sedation that
- prevents pain entirely.
- ..
- Made a joke for Debra to err on the high side because I was absent the
- day tricks to tolerate pain were taught in spy training class.
- ..
- Jessica appoligized that the OR and ICU are busy this evening, so
- debridement surgery must be done in the ER without sedation, because
- the ER does not have monitoring equipment required for conscious
- sedation. She gave an alert this procedure may be painful, hopefully
- eased by hydromorphone.
- ..
- Jessica said she will try to minimize pain by making multiple
- injections around the perimeter of the wound. Each injection will
- hopefully numb the area sufficiently to reduce pain from the next
- injection.
- ..
- The doctor then tied her hair on top of her head, and placed a mask
- over her face.
- ..
- The nurse, Debra was kind enough to place her hand on my shoulder.
- ..
- Jessica was very considerate giving a verbal warning to grasp the
- sides of the gurney for a brace to accept the first injection of local
- anesthetic.
- ..
- The point of penetration seemed like pain level "20". Was difficult
- to avoid responding to this pain.
- ..
- Told Jessica pain level 20 is a new experience having to hold the body
- still under seemingly tortuous cutting on the leg. Appoligized for
- not entirely succeeding.

- She said that's okay, and that she would pause for the patient to

- pausing between each. Fortunately, pain quickly subsided from 20 to
- 0 between pauses.
- ..
- Jessica explained she was removing infected tissue from the wound,
- and asked if there was any feeling. At first said no, but then pain
- increased dramatically. At that point, Jessica said she would do
- another injection of local anesthetic.

- While Jessica worked, Jessie or someone spoke to her several different
- times through curtain closing the cubical, and asked if Jessica could

- patient, and very grateful for Jessica's professional, thorough
- surgical care. The hardest part of the exercise was to hold the body
- still rather than react to pain. The right leg actually jumped
- several times despite gripping the sides of the gurney with both
- hands.
- ..
- At one point, made a joke requesting a leather bite block, like you
- see in the movies of surgery without sedation, for example during the
- civil war. Debra was quick witted, saying they are short on bite
- blocks, but there is plenty of whisky, if that will help??
- ..
- Finally, Jessica said "I think we've got it. There will be no more

- had been done later after the OR or ICU could accomodate this
- procedure?
- ..
- Jessica indicated she "just works here." She feels the important
- thing now is to concentrate on good wound care for speedy recovery to
